Care & Follow-Up

Chronic conditions require ongoing monitoring and follow-up.

Patients are expected to:

  • Follow the agreed plan of care
  • Attend recommended follow-up visits
  • Complete required labs and monitoring

Typical follow-up timelines:

  • ADHD: every 4 weeks
  • Hormone therapy: every 6–12 weeks initially, then every 3–6 months
  • Chronic conditions: every 1–6 months

Patients must be seen at least once every 12 months to remain active.

  • Medical concerns, medication changes, or new issues require an appointment
  • Portal messaging is for administrative questions or clarification only

Concierge members may have enhanced access per membership agreement, but visits are still required for complex concerns.

  • Insurance is billed as a courtesy
  • Coverage is not guaranteed

Patients are responsible for:

  • Co-pays, deductibles, co-insurance
  • Non-covered services
  • Remaining balances

Some services may not be covered. Membership plans follow separate agreements.

For urgent symptoms such as chest pain, severe shortness of breath, or suicidal thoughts:

  • Seek immediate medical care or call emergency services.
  • This practice does not provide emergency care.

Appointments & Scheduling
  • 24-hour notice is required for cancellations
  • Missed or late-canceled visits are tracked

Repeated missed appointments may result in:

  • Limited scheduling access
  • Possible discharge from the practice

Arriving more than 10 minutes late may require rescheduling.

Medicare/Medicaid patients are not charged fees but remain subject to scheduling policies.

  • Medications are prescribed as part of a shared plan of care.

    • New medications require follow-up before refills
    • Refills require up-to-date visits and monitoring
    • Early refills may not be granted

    Controlled substances require strict follow-up and compliance.

Care is provided only to the scheduled patient.

  • Family members must have their own appointments
  • Additional patients cannot be added to a visit

All notes are reviewed and finalized by your provider.
